Graft survival and density

Summary

Grafts secure within about ten days and the result takes twelve months to show. Almost everything that decides survival happens in the few hours of the procedure itself, which is why the questions worth asking are all about how the day is run.

1. The timeline.

Grafts are held by fibrin and early clotting within hours, are reasonably secure at seven to ten days, and are essentially permanent after that. Washing and sleeping instructions in the first week exist for that window specifically.

Then the transplanted shafts shed, usually between weeks two and eight. The follicle stays; the hair falls. Month three commonly looks worse than the day after surgery, which surprises people who were not warned.

New growth appears from month four, thickens through months eight to twelve, and the crown can take eighteen.

2. What decides survival.

Time outside the body. Follicles deteriorate once extracted. A session that runs very long, or extracts far more grafts than can be implanted promptly, risks yield.

Handling. Crushing a graft with forceps damages it. This is a skill question, and in a high-volume clinic it is a question about the technicians rather than the surgeon.

Channel quality. Channels that are too large, too deep or badly angled compromise both survival and appearance.

Blood supply. Packing grafts too densely in one pass can outstrip the recipient area's ability to feed them.

3. Why nobody promises full density.

Native scalp carries roughly 80 to 100 follicular units per square centimetre. Transplants typically achieve 30 to 45, and higher packing raises the risk of poor survival rather than guaranteeing a better result.

This is not a limitation of any particular clinic. It is why a good result reads as a natural head of hair rather than as the density you had at twenty, and why a clinic promising the latter is describing marketing rather than surgery.

4. What this makes worth asking.

How long the session will run and whether it is split across days; how many patients the clinic runs concurrently; who holds the instruments during extraction and implantation; and what density the surgeon is planning for your recipient area.

None of these appear on a price list, and all four bear directly on whether your grafts survive.

Common questions

How long until hair transplant grafts are secure?

Reasonably secure at seven to ten days. The washing and sleeping instructions in that first week exist for exactly that reason.

Why did my transplanted hair fall out?

Shock loss is normal. The shafts shed between weeks two and eight while the follicles stay and regrow from month four.

When will I see the final result?

Most of it at twelve months, and up to eighteen for the crown. Any photograph taken before nine months is showing an early stage.
